Face to Face Verification Form for Extended Validation (EV) and Code Signing Certificates Print

Face to Face Document:

Use the attached form when a face to face verification is required to validate a non-incorporated business entity for an EV or Individual Code Signing Certificate.
Note:  The face-to-face verification form should be filled and signed by a Notary which is authorized to conduct business in your area/country


You can apply for an OV Code Signing certificate as an individual.
In addition to the Face to Face document, you will need the following: 

  1. Copy of a valid driver's license or passport of the Applicant
  2. Copy of a recent major utility bill (i.e. power bill, water bill, etc.) or bank statement of the Applicant

Note:  If the Drivers License and Passport do not show the current address or those details do not match the account, you can submit one of the documents listed above (1.) and 2 documents from (2.)
           
The documents must be recent.  Dated within the last 6 months.

  • Once the documents have been submitted, our validation team will need to complete callback verification:
  • Callback to a Verified Telephone Number (to verify an applicant) found within an online regional directory of the applicant's location.


Non-incorporated Business Entity applicants for Extended Validation (EV) Certificates

A "Business Entity" comprises the following options:

  • General partnerships
  • Unincorporated associations
  • Joint ventures
  • Sole proprietorships

Note To complete validation of an Extended Validation (EV) certificate, the following documents are required (in addition to the Face to Face document):

  1. Government issued photo ID [driver's license OR passport]
  2. One Financial institution documents [A bank statement or credit card less than six months old]
  3. One Non-Financial documents [Gas bill, Water bill, Power bill ]
  4. Business entity registration with listing company name


Once you have submitted the Face-to-Face document, our validation team will verify the Notary's credentials with the respective registration agency.
